DESCRIPTION
The Registered Nurse Specialist will provide services to a caseload of children with special health care needs under the supervision of a Senior Registered Nurse Supervisor. The CMS nurses primary role in this position is to provide care coordination which includes arranging for services with the childs physicians and other health care providers. The nurse will identify physical, developmental and psychological needs of the child by completing a written assessment; offering education and disease management information to the child and family, create an individualized plan of care; identify appropriate community programs and services; and communicating with teachers and school staff to provide comprehensive care coordination based on the childs needs.
EXAMPLES OF WORK
Care Coordination duties include but are not limited to the following:
1. Initiates and maintains contact with a caseload of special needs children and their families through: a) Completing a comprehensive nursing history and assessment; developing goals and objectives for care; and evaluating and revising plans of care. b) Coordinating social, health and economic community resources for the family. c) attending home and school visits as needed. d) Providing health education and anticipatory guidance regarding growth and development to the child and family. e) Obtains and reviews medical records from various providers. f) Provides transitioning education to young adults and their families to adult systems of care.
2. Specifically related to (CMAT) Childrens Multi:Disciplinary Assessment Team Program: Serves as core member of CMAT team, providing administrative support to the Medical Director. Functions as lead for clinical information provided to team at the time of staffing. Ensures that all post staffing required documentation is completed and submitted per program guidelines. Ensures that team meetings are scheduled in accordance with review schedule for each child. In the absence of CMAT Social Worker ensures that monthly (PASSAR) Pre:admission Screening and Resident Review report is submitted in timely manner. Completes discharge from CMAT program per program standards when services are no longer required. Facilitates transition to routine care coordination as necessary.
3. Specifically related to MFC Program: Is cross trained for, and functions as nurse care coordinator for Medical Foster Care Team in the absence of MFC Nurse. Performs on:call duties on a 24 hour rotating basis. Provides Level of Reimbursement Training for all new MFC team members. Performs other duties as required by Nursing Supervisor or Director of Nursing.
